Be advised that Fort Worth Heart will be migrating to a new Electronic Health Record system August 25, 2025. We appreciate your cooperation and patience with all our team members during this transition, as we attempt to make this as seamless a process as possible for you.

Please allow for additional time during check-in, as all patients will be required to fill out new paperwork, present identification and insurance cards (if applicable) under our new E.H.R. system. There will be a new patient portal available with our new system, and our team members will help with enrollment during check-in.

All your records will continue to be available with Fort Worth Heart.
