Be advised that Fort Worth Heart migrated to a new Electronic Health Record system on August 25, 2025. We appreciate your cooperation and patience with all our team members during this transition, as we attempt to make this as seamless a process as possible for you.
Please allow for additional time during check-in, as all patients will be required to fill out new paperwork, present identification and insurance cards (if applicable) under our new E.H.R. system.
There is a new patient portal available with our new system, which allows you to check-in for appointments from home, update your information, print your records, communicate with your provider, and much more! For more information on our new patient portal please look under our Patient Info tab, or our staff can help sign you up at check-in.
